Livewell is a healthy lifestyle service for Derby residents. It offers free 12-month intervention programmes to support adults and children make lifestyle changes to improve their health and wellbeing.

Our variety of programmes includes weight management for children and adults, stop smoking and community-based NHS Health Checks.

Livewell is proud to employ 25 of the most dedicated and experienced behaviour change individuals and we’re passionate about giving your patients a high-quality service.

Referral criteria

Adults and children registered to a Derby GP practice:

Smokers – young people aged 12-17 and adults
Children on the 85th percentile or higher for Live IT weight management
Adults with a BMI of 30 or higher (27.5 for South Asian ethnicity)
Adults aged 40-74 without pre-existing conditions for NHS Health Checks

How to refer

Health and other professionals can signpost patients/people to our website to apply to join our weight programmes. Patients can also apply and book an NHS Health Check or their first stop smoking appointment online too.

Professionals can now refer patients/persons in your care through our online join forms or through downloadable forms for the following programmes:

Liveability (weight management programme for people with learning disabilities).
Live IT/Kick IT form (supporting children to manage weight/stop smoking).
